Residential Habilitation:

Currently Residential Habilitation services are being provided by Onondaga Community Living with their guidance and mentorship. This service is divided up into three separate teams of staff and people supported. As funds are raised, each individual team will move on to the new agency of Connections of Central New York, Inc.

Residential Habilitation is a support service available through the Medicaid Waiver to individuals either living alone or with others. The number of hours of service provided varies based on individual needs and desires. The goal of this service is to support individuals in developing skills of daily living so they may live as independently as possible in the community of their choice. In our planning, we encourage involvement from all who know the individual best. We value input from family and friends recognizing that natural supports are the foundation of an enriching life.

Medicaid Service Coordination:

Currently Medicaid Service Coordination is being provided by Onondaga Community Living with their guidance and mentorship and will be the first team of staff and people supported that will begin Connections of Central New York, Inc.

Medicaid Service Coordination offers support, advocacy, referral, and service planning. It is available to assist individuals with identifying and choosing supports which are important to the success of their goals and desires. Information and support is given to assist in making informed choices. This service is developed in partnership with the individual, their family, or other people involved.
